MetLife’s PPO plan offers you freedom and flexibility to select any in-network dentist, and offers extensive coverage for nearly any dental service.
The PPO plan uses a coinsurance model where different services are covered based on their “category.”
Type A - Preventative

Type A refers to basic dental care, such as: oral exams, cleanings, and x-rays. Type B refers to things like: sealants, root canals, and fillings. Type C refers to more intensive treatments like crowns, dentures, and bridges. Type D refers to braces, retainers, and other orthodontics.
The PPO plan has a $50 deductible (this deductible does not count for preventive and diagnostic services when visiting an in-network provider) and an annual maximum of $2,250. That means you can only receive up to $2,250 of coverage per year. There is also a lifetime orthodontic coverage of $2,000. After reaching your $2,000 limit, all orthodontic coverage stops and you will need to pay out of pocket.
